In theBroken Aqueductregion, climb up the small hill on the eastern side of the structure where the Ancient Memory is found. The hammer is stuck in the head of a Maegfolc.

Just past the large Essence Generator-blocked door in theMaegfolc Graveyard, you’ll find another generator-blocked chamber. The spear is inside the chamber, in the sarcophagus at the center of the room.
